CMB Group Order Form

Our Group Order Form is a simple way to send us large orders with many shipments. Please use it when you are shipping to 5 or more addresses.

The form is an Excel spreadsheet in which there is a line per gift recipient. Before using the order form, please review the instructions below.

The Excel File and Instructions can be opened directly from the links below. When you open the Excel File, first save it to your local drive. The Zip file below contains both files. Please call or use our contact us form if you have any questions about the process.

When you have completed your Group Order form, use our Uploading Your Group Order Form to send us the file.

After you upload or email us your group order, we will respond within 24 hours. If you do not get a response from us please contact us via phone (888-727-7887) or use our Contact Us Form.
